In-Home Clinician PROGRAM:
Intensive Home Based Therapy program GENERAL STATEMENT OF
Responsibilities:
IICAPS Clinicians provide intensive home-based services to children at risk for psychiatric hospitalization.
Services provided include individual and family therapy, parenting skills training, crisis support, extensive case management, and advocacy.
IICAPS operates from a team approach and consists of a Clinician and Mental Health Counselor, both of whom participate in sessions and treatment meetings.
A treatment plan tailored to the child and family is developed and implemented by the two-person team.
EXAMPLES OF
Responsibilities:
To provide comprehensive evaluations, treatment plans and treatment of children, adolescents and families.
This includes evaluation/assessment; home and community based services; outreach; case and care management; individual, group and family therapies, as well as crisis intervention.
To implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation (only as per agency policy) advocacy and linkage to other needed services.
To provide community based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to:
client homes, agency sites, schools, emergency rooms, shelters, police stations, etc.
The majority of treatment services are provided in the home of the client and will take place primarily in the late afternoon, evenings, overnight and on weekends.
To work a flexible schedule as based on the needs of assigned families.
To carry a pager and to be available to provide after-hour emergency mobile (in-home, community, emergency room, school-based, other) services including both phone and face to face interventions, as based on agency and/or client needs Maintain contact and provide follow-up with families requiring this service.
This will include daily home visits and home based therapies.
Maintain confidential and/or any other required program documents.
Maintain a level of productivity as determined by the Program Coordinator and assigned by the program supervisor.
To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
To engage families in creative and productive service negotiation and planning.
To participate in Child Specific Team case planning and conferences.
To facilitate the greatest possible level of family involvement in all facets of program service delivery and decision making.
To assist in the organization, marketing, planning and implementation of the program.
Actively participate in trainings and staff meetings.
To serve on agency and/or community committees.
Actively participate in supervision and report directly to the supervisor.
To supervise assigned staff and/or student interns.
Other such duties as assigned by the Supervisor.
HP1 EDUCATION &
Experience:
A Master's Degree in Social Work, Psychology/Clinical Mental Health Counseling, Marriage and Family Therapy or related human services field.
Must have a CT license as a mental health provider at the associate level (i.
e.
,:
LMSW, LPCA, LMFTA); or licensed or license-eligible psychologist in Connecticut.
Experience:
2 years of post-graduate clinical experience working with early school-aged children or families is preferred.
Experience in child development theory, family therapies, as well as family systems theory and practice, is preferred.
Skill to maintain a professional, confidential work environment Ability to manage multiple tasks at one time in an efficient manner Ability to relate to children and families in a positive manner Ability to establish and maintain appropriate boundaries with children and families Valid driver's license with good driving record, reliable transportation and proof of auto insurance.
HOURS, COMPENSATION &
Benefits:
